I just published the STL's for my Dual E3D mount on Thingiverse.
http://www.thingiverse.com/thing:373772
Hope it's useful to you guys. It took a bit of work ironing out the kinks. If you have any suggestions or improvements, please let me know, I'll see what I can do (though, I can't promise it won't take me a very long time!

).
My final step now is to figure out the offsets (will only need to be offset in the X-Axis). It actually adjusts quite well though.

Very nice, thank you. What's the best orientation to print? And support is required?

Here's the orientation that gave me the best results.

Yes to supports... in Cura, I use Lines with 40% Fill, 0.7mm distance from X/Y, 0.1mm distance from Z.
I also updated Thingiverse with the file for all the parts oriented. Personally, I had better results printing them individually though... more accurate.

Thank you for the part, it printed nicely and I have both my pieces mounted now. My Y offset on my rostock set in the eeprom to 1014 (extr1) and -1014 steps (extr2). Is that about what you have set?
Also how did you calculate your horizontal radius. I was perfectly calibrated with a single extruder but now with two I am trying to figure out how to calibrate.
When I run g0 z0 x0 y90 f3500 my extr2 touches the glass before the extr1 can be adjusted close enough.
when I run the below two commands the extr2 is higher than extr1
g0 z0 x-77.94 y-45 f3500
g0 z0 x77.94 y-45 f3500
