Yes the Diamond would need to be above the effector with the nozzle poking through a hole in the centre.

I don't think you should, because E3D is kinda most widely-used. It's just me who want to use Diamond hotend with it, and my lack of skills to alter open source hardware except for mechanics makes me sad.

... can define several tools with different mix ratios if you wish. It's probably best to use firmware retraction, so that all extruder drives retract the same amount regardless of the mix ratio. I am told that this works well for fhe Diamond hot end. Use M207 to define the firmware retraction parameters.

... t that hard to duplicate either, should you need it. It requires one of the spare analog pins on the Arduino. The limiting factor on all these simple expansions are the unused pins from the Arduino. I can't help you with operational details on the Diamond. I only run single filament hotends myself.
