Hotend cannot get to 240

I have a V2, just upgraded to the standard 12V power supply from the old ATX psu but I'm still having issues with heating. I ran the PID tuning and when I tried to preheat to 240, the temperature stopped climbing around 228 and I got the decoupled error message.

I'm thinking the retraction distance is too high period. The HE280 (Given you're a new poster with new issues, I'm assuming a V3), is based off the E3D Geometry to an extent, and any retractions above 2mm are potentially problematic with both. Print quality issue

The calibration cube printed great, no blobs or gaps/holes. The extrusion multiplier came out to .86. Next I printed the fan shroud but I didn't get anywhere close to the same print quality as the calibration cube. Is there a permanent solution to this? I'm getting this everytime I turn on my V2 and only way to clear it up is to disconnect and reconnect in MatterControl.

If you connect the wires together that will create a dead short and surely blow a fuse. You are connecting a positive to a negative, a real no-no and if the fuse wasn't there for protection the wires would burn and possibly cause a fire.
